Saudi MOH newborn screening is not one-and-done in the NICU
The 2024 MOH pathway means one routine specimen for most newborns, two for infants under 37 weeks or under 1500 g without day-one transfusion, and four when day-one red-cell transfusion changes the schedule.
THE POLICY IN ONE LINE
Saudi newborn screening is a one-specimen pathway for most newborns, but it is not one-and-done for every NICU admission. The Ministry of Health's 2024 guide specifies two routine specimens when gestational age is under 37 weeks OR birth weight is under 1500 g without a red-cell transfusion during the first day, and four when that day-one transfusion changes the schedule.
This procedure must be performed within 24 to 72 hours after birth.
THE STANDARD PATHWAY
For a standard newborn, collect the first heel-prick dried-blood-spot specimen at age 24-72 hours and before discharge. The current MOH page adds a fallback: if collection was not possible in the first 72 hours, the specimen may be collected within the first 28 days. That is a fallback window, not the routine schedule.
| Pathway | Trigger | Routine timing | Total |
|---|---|---|---|
| Standard newborn | At least 37 weeks AND at least 1500 g | Specimen 1 at age 24-72 h, before discharge | 1 |
| High risk, no day-one red-cell transfusion | Under 37 weeks OR under 1500 g | 1: age 24-72 h. 2: just before discharge if discharge is before 3 wk; otherwise at age 3-4 wk | 2 |
| High risk, day-one red-cell transfusion | Under 37 weeks OR under 1500 g, with red cells during the first day | 1: before transfusion. 2: age 24 h-7 d. 3: before discharge if before 3 wk, otherwise age 3-4 wk. 4: 4 mo after transfusion | 4 |
